ACMC Anticoagulation Clinic Receives National Award
ACMC has earned national recognition for our Anticoagulation
Program with the Dare to Reach Excellence in Anticoagulation Management
(D.R.E.A.M.) award. It's only the second time that a Minnesota medical group has received this
honor. This award was given by Roche, an international manufacturer
of pharmaceuticals and diagnostic equipment.
How our anticoagulation program works
ACMC's Anticoagulation Clinic manages over 2,000 patients at our 10 regional
multispecialty medical clinics. The medications used are called anticoagulants.
The medications require close monitoring and continuing patient education to
avoid "over anticoagulation" which can result in bleeding, and "under
anticoagulation" which can cause harmful blood clots. The clinic assists
the patient and their physicians in monitoring people on anticoagulants,
provides individualized education and nutritional counseling and teaches
self-management tools for safe usage.
It began with the formation of a multi-specialty group
The initiative began in 2000 as a joint effort among physicians,
the clinic's laboratory and the Health Learning Center. The program was moved to a point-of-care service that's the equivalent
of one-stop shopping. Patients can come in, have their finger pricked, receive
the results in seconds, and have their medication adjusted if necessary, all
in one visit.
A look at our program results
Our efforts have resulted in 80-90% of patients treated through the anticoagulation
services at ACMC being successfully controlled compared to national averages
near 70th percentile for those with established programs and in the upper 40th
percentile for those without a structured program.
What makes our program so successful?
Jo DeBruycker, RN, and manager of the Health Learning Center, credits
the program's success to a multidisciplinary team that works to design and implement this
initiative. "This program wouldn't be what it is today without
people from all aspects of ACMC lending their support and expertise," she
said. "We are a team with one goal, delivering the best possible care
for our patients."
