2008 Sports Physical Schedule

Affiliated Community Medical Centers would like to invite you to attend one of the upcoming 2008 Athletic Physicals scheduled in your community. The cost of the athletic physical is $30.00, and is payable at the time of the exam. Insurance will not be filed for these exams. Please be sure to bring a completed copy of the Minnesota State High School League Sports Qualifying Exam form with you. This form will also be available at all of our clinics. It is important that this form be signed by a parent or guardian and brought with you at the time of the exam. No athletic physicals will be performed without a completed and signed form.

The cost of the exam is $30.00 and is payable at time of the exam. Insurance will not be filed for these exams.
