Yes, because insurance reimbursement rates vary, you will need a referral from your physician.

Who will I be meeting with at the Diabetes Center?
Our outpatient diabetes program is led by certified diabetes educators. Through our
comprehensive program you will meet with clinical nurse specialists, nurse educators, and dietitians who specialize in the management of diabetes.

Will I need to be seen again by my health care provider once I've been seen in the Diabetes Center?
Yes, your health care provider may want to see you back in three to six months. The Diabetes Center will keep your provider informed on how we have been working with you.

Will the Diabetes Center staff make referrals for additional services?
Yes, referrals for services such as exercise physiology and counseling services can be
facilitated by Diabetes Center staff.

  • Exercise Physiology - Patients may benefit from this service if they are unsure of what exercises they can safely do with diabetes, heart conditions, or other health problems. Physical activity is an important part of diabetes management.
  • Counseling Services - Some patients may need additional emotional support in order to give their diabetes the attention it needs. This may help with keeping their life in balance with both physical and emotional health.
