Stocks/Securities
Gifts of appreciated securities may offer substantial tax advantages including forgiveness of tax on capital gain and an income tax charitable deduction at the full market value of the securities. If you like to know more information about stock transfer, please fill out and return the ACHF Contribution Form. (Please leave the Cash/Check and Credit Card fields blank and check the "contact me about other contributions" box on the bottom.)

Memorials and Tribute Gifts
Gifts in memory of a family member or friend, other individuals or in recognition of a special occasion express what words alone cannot. Please make your check payable to Affiliated Community Health Foundation and include the name of the person or occasion you would like to honor. Please include the address of the family so that we may notify them of the gift.
